Step 1: Call 310-451-8880 to Schedule Your Appointment

Step 2: Complete the New Patient Forms Below

  • New Patient Intake Form
    1. Our Patient Care Coordinator will invite you to complete the intake form electronically when you call our office to schedule your first appointment
    – OR –
    2. Print it out, complete it, and bring it with you to your first visit. It is also helpful to bring copies of past medical records to support your medical history
  • Consent to Request or Release Records (2 Options)
    1. Print it out, complete it, and bring it with you to your first visit to have your medical records sent to Akasha Center.
  • 2. Print it out, complete it, and send to us to have your medical records sent from Akasha Center to another practitioner.
